Federal Form Links for Employees

 

The following links all provide search options for nearly every agency form that you might need. Each site is set up differently depending on your search criteria.

Individual forms will not be posted to this website. Directing you to the following sites will insure that the most current form version and downloadable options are available to you.

  • OCIO Forms  The approved computer generated forms on the OCIO site center are available in a variety of formats.

  • GSA Forms GSA's forms library provides you with several convenient formats to chose from depending on your requirements. DocNet forms offer you the ability to fill them in on the screen, then save, print, fax or Email directly from the Internet. Other forms are designed for use with other programs such as FormFlow, PDF, etc.

  • OPM Forms The OPM Website offers many Standard Forms (SF), Optional Forms (OF), Office of Personnel Management (OPM) Forms, Federal Employees Group Life Insurance (FE) Forms, and Forms issued by the Retirement and Insurance Service (RI). These are available in the following formats: FormFlow, Adobe Acrobat PDF Image, Screen Fillable Adobe Acrobat, FormNet, Microsoft Word, and WordPerfect.

  • Fedforms  Contains forms needed for the top 500 Government services used by the public. FedForms does NOT contain every form used by Federal agencies and does not contain instructions or program descriptions. If you do not find the form you are looking for in FedForms, you can either click on the "Agency Forms Management Web Sites" link or do a search in FirstGov."
